Waiver
You sign this once, before your first paid session, with your typed name. It covers training at gyms, homes, parks and online. We keep only your typed name, when you signed and the version you signed — no health questions.
KRAVE4FITNESS — ASSUMPTION OF RISK, WAIVER AND RELEASE OF LIABILITY
Please read this document carefully. It affects your legal rights.
1. Who and what this covers. This agreement is between you and Resha Green and Krave4Fitness (together, "Trainer"). It applies to every training session, class, camp, or activity you take part in with the Trainer, in any place, including gyms, your home or another private residence, parks and other outdoor locations, and online or virtual sessions.
2. Fit to exercise. I confirm that I am physically able to take part in exercise. I have consulted a doctor if I have any doubt about whether exercise is safe for me. I will tell the Trainer in person about anything she should know before or during a session. I understand that the Trainer is not a medical provider and that I take part voluntarily.
3. Assumption of risk. I understand that exercise involves risks, including muscle strains and sprains, falls, injury from equipment, injury from the surface or surroundings, weather and temperature, and, in rare cases, serious injury or death. I knowingly and voluntarily accept these risks.
4. RELEASE OF LIABILITY, INCLUDING THE TRAINER'S OWN NEGLIGENCE. I RELEASE, WAIVE, AND DISCHARGE THE TRAINER, RESHA GREEN, KRAVE4FITNESS, AND THEIR HEIRS, EMPLOYEES, AND AGENTS FROM ALL CLAIMS, DEMANDS, AND CAUSES OF ACTION FOR INJURY, ILLNESS, DEATH, OR LOSS OF OR DAMAGE TO PROPERTY ARISING FROM MY PARTICIPATION, EVEN IF CAUSED BY THE NEGLIGENCE OF THE TRAINER. I UNDERSTAND THAT THIS MEANS I AM GIVING UP THE RIGHT TO SUE THE TRAINER FOR HER OWN NEGLIGENCE.
5. Indemnity. I agree to cover the Trainer for losses and costs she incurs because of a claim brought by me or on my behalf that this agreement releases.
6. Property and locations. Sessions may take place at a facility or property that someone else owns or controls. I will follow that location's rules. I am responsible for my own belongings.
7. Adults only. This agreement is for people aged 18 or older. I confirm that I am at least 18 years old.
8. Everything else. If any part of this agreement is found to be unenforceable, the rest stays in effect. This agreement is governed by the laws of the State of Texas. I have read this agreement, I understand it, and I sign it voluntarily. A typed name and a checked box serve as my signature.
